Tacrolimus Ointment
Usage
Tacrolimus ointment is a topical immunomodulator used in the treatment of inflammatory skin conditions, particularly when conventional therapies (such as topical corticosteroids) are ineffective or not suitable.
It is commonly prescribed for:
• Atopic dermatitis (eczema) – moderate to severe cases
• Sensitive skin areas (e.g., face, eyelids, skin folds) where corticosteroids may be less suitable
• Steroid-sparing therapy for long-term management of inflammatory dermatoses
• Other practitioner-directed inflammatory skin conditions
Tacrolimus ointment helps reduce inflammation, itching, and redness associated with eczema and similar conditions.
Dosage
Dosage and frequency should be determined by the prescribing practitioner based on the severity and location of the condition.
Typical dosing considerations include:
• Apply a thin layer to the affected area twice daily (morning and evening), or as directed
• Use the smallest amount necessary to control symptoms
• Treatment may be reduced or discontinued once symptoms improve
• For maintenance therapy, application frequency may be reduced (e.g., twice weekly) under medical guidance
Hands should be washed after application, unless the hands are the treatment area.
Mechanisms of Action
Tacrolimus is a calcineurin inhibitor that suppresses immune-mediated skin inflammation.
Its mechanisms include:
Inhibition of calcineurin
Tacrolimus binds to intracellular proteins and inhibits calcineurin, a key enzyme in T-cell activation.
Reduction of inflammatory cytokines
This leads to decreased production of pro-inflammatory cytokines, reducing immune response in the skin.
Suppression of T-cell activation
By inhibiting T-cell activation, tacrolimus reduces inflammation, itching, and skin damage.
Unlike corticosteroids, tacrolimus does not cause skin thinning, making it suitable for sensitive areas.
Contraindications
Tacrolimus ointment should not be used in patients with:
• Known hypersensitivity to tacrolimus or any component of the formulation
• Active skin infections at the application site unless appropriately treated
• Certain immunocompromised conditions, unless under specialist supervision
Precautions
Tacrolimus ointment should be used with caution in patients with:
• Compromised immune systems
• Skin infections (bacterial, viral, or fungal)
• Extensive or severely damaged skin areas
Additional precautions include:
• May cause burning, stinging, or itching, especially during the initial days of treatment
• Avoid excessive sun exposure or UV therapy, as tacrolimus may increase skin sensitivity
• Do not apply under occlusive dressings unless directed by a healthcare professional
• Long-term safety should be monitored, particularly with prolonged or extensive use
Patients should seek medical advice if they experience persistent irritation, worsening symptoms, or signs of infection.
Storage
• Store below 25°C unless otherwise directed
• Keep the tube tightly closed when not in use
