Trilostane Capsules
Usage
Trilostane capsules are an oral medication used primarily in veterinary medicine for the management of hyperadrenocorticism (Cushing’s disease). It works by reducing excessive production of adrenal hormones.
They may be prescribed for:
• Cushing’s disease in dogs (pituitary-dependent or adrenal-dependent)
• Occasionally in cats (off-label use)
• Other practitioner-directed endocrine disorders involving excess cortisol production
Compounded capsules allow for individualised dosing tailored to the animal’s needs.

Dosage
Dosage should be determined by the prescribing veterinarian based on the animal’s weight, condition, and response.
Typical dosing considerations include:
• Administered once or twice daily, depending on treatment plan
• Dose is typically weight-based
• Given with food to improve absorption
• Dose adjustments are guided by clinical response and monitoring tests (e.g., ACTH stimulation tests)
Regular monitoring is essential to ensure safe and effective therapy.

Mechanisms of Action
Trilostane is an adrenal steroid synthesis inhibitor.
Its mechanisms include:
Inhibition of 3β-hydroxysteroid dehydrogenase
Trilostane blocks an enzyme required for the production of cortisol and other adrenal steroids.
Reduction of cortisol production
This decreases excessive cortisol levels associated with Cushing’s disease.
Regulation of adrenal function
Helps restore a more normal hormonal balance, improving clinical signs such as excess thirst, urination, and appetite.

Contraindications
Trilostane should not be used in:
• Animals with hypersensitivity to trilostane or formulation components
• Pregnant animals
• Animals with primary hepatic disease (unless under strict supervision)
• Patients with Addison’s disease (hypoadrenocorticism)

Precautions
Trilostane should be used with caution in:
• Animals with renal or hepatic impairment
• Animals with anaemia or electrolyte imbalances
• Patients receiving other medications affecting adrenal function
Additional considerations include:
• Risk of over-suppression of cortisol, leading to hypoadrenocorticism (Addisonian crisis)
• Possible side effects include:
• Lethargy
• Vomiting or diarrhoea
• Reduced appetite
• Regular monitoring is essential:
• ACTH stimulation tests
• Electrolytes
• Clinical signs
• Treatment may require dose adjustment or temporary discontinuation
Owners should seek veterinary advice immediately if they observe weakness, collapse, vomiting, or diarrhoea.

Storage
• Store below 25°C unless otherwise directed
• Keep capsules in a tightly closed container
• Protect from heat, moisture, and direct light
• Keep out of reach of children and animals
Follow the storage instructions provided on the dispensing label supplied by the pharmacy.
