Trazodone Capsules
Usage
Trazodone capsules are an oral medication classified as a serotonin antagonist and reuptake inhibitor (SARI), used primarily for depression and commonly for sleep disturbances due to its sedative properties.
They may be prescribed for:
• Major depressive disorder
• Insomnia (off-label use)
• Anxiety-related conditions
• Other practitioner-directed psychiatric indications

Dosage
Dosage should be determined by the prescribing practitioner based on the condition and patient response.
Typical dosing considerations include:
• Often administered once daily at bedtime for sleep-related use
• May be given in divided doses for depression
• Usually started at a low dose and gradually increased
• Capsules should be taken with food to improve absorption and reduce side effects
Patients should take trazodone as prescribed and avoid abrupt discontinuation.

Mechanisms of Action
Trazodone has a multimodal antidepressant mechanism.
Its mechanisms include:
Serotonin reuptake inhibition
Trazodone weakly inhibits the reuptake of serotonin (5-HT), increasing its availability.
Serotonin receptor antagonism
Blocks certain serotonin receptors (e.g., 5-HT2A), which may reduce anxiety and improve sleep.
Histamine receptor blockade
Contributes to sedative effects.
Alpha-adrenergic blockade
May contribute to its effects on sleep and blood pressure.

Contraindications
Trazodone should not be used in patients with:
• Hypersensitivity to trazodone or any component of the formulation
• Concurrent use of monoamine oxidase inhibitors (MAOIs) or within the recommended washout period

Precautions
Trazodone should be used with caution in patients with:
• History of suicidal ideation, particularly in younger patients
• Cardiovascular disease
• Hepatic or renal impairment
• History of seizures
Additional considerations include:
• Common side effects include:
• Drowsiness or sedation
• Dizziness
• Dry mouth
• Risk of orthostatic hypotension (drop in blood pressure on standing)
• Rare risk of priapism (prolonged erection requiring urgent medical attention)
• Risk of serotonin syndrome when combined with other serotonergic agents
• May impair ability to drive or operate machinery
Patients should seek medical advice if they experience worsening mood, fainting, or prolonged erection.

Storage
• Store below 25°C unless otherwise directed
• Keep capsules in a tightly closed container
• Protect from heat, moisture, and direct light
• Keep out of reach of children and pets
Patients should follow the storage instructions provided on the dispensing label supplied by the pharmacy.
