Testosterone Cream
Usage
Testosterone cream is a compounded transdermal hormone therapy used to treat testosterone deficiency (hypogonadism) and related symptoms. It may be prescribed for both men and women under specialist supervision.
It may be prescribed for:
• Male hypogonadism (low testosterone levels)
• Low libido or sexual dysfunction
• Fatigue, reduced energy, and decreased muscle mass
• Hormonal imbalance in women (e.g., low androgen levels)
• Part of individualised hormone replacement therapy (HRT) programmes
Dosage
Dosage should be individualised and prescribed by a qualified healthcare professional based on hormone levels and clinical response.
Typical dosing considerations include:
• Apply a measured amount once daily, or as directed
• Apply to clean, dry skin (e.g., shoulders, upper arms, thighs, or other recommended sites)
• Rotate application sites to minimise skin irritation
• Allow the cream to dry before covering with clothing
• Wash hands after application
Regular monitoring of testosterone levels is required.
Mechanisms of Action
Testosterone is an androgen hormone.
Its mechanisms include:
Binding to androgen receptors
Testosterone binds to receptors in target tissues, influencing gene expression.
Development and maintenance of male characteristics
Supports muscle mass, bone density, and libido.
Conversion to active metabolites
Testosterone may be converted to:
• Dihydrotestosterone (DHT) – important for androgenic effects
• Estradiol – contributes to bone and metabolic functions
Systemic absorption via skin
Transdermal delivery allows testosterone to enter circulation and maintain stable hormone levels.
Contraindications
Testosterone cream should not be used in patients with:
• Hypersensitivity to testosterone or formulation components
• Known or suspected prostate or breast cancer (in men)
• Pregnancy or breastfeeding
• Severe cardiac, hepatic, or renal disease (as determined by the practitioner)
Precautions
Testosterone should be used with caution in patients with:
• Cardiovascular disease or risk factors
• Prostate disorders
• Polycythaemia (elevated red blood cell count)
• Liver impairment
Additional considerations include:
• Risk of transference to others via skin contact:
• Avoid skin-to-skin contact until fully absorbed
• Cover application site if necessary
• Possible side effects include:
• Acne or oily skin
• Hair growth or hair loss
• Mood changes
• Fluid retention
• In women, excessive exposure may cause androgenic effects (e.g., voice changes, hirsutism)
• Regular monitoring of:
• Hormone levels
• Haematocrit
• Prostate health (in men)
Patients should seek medical advice if they experience shortness of breath, swelling, mood changes, or signs of hormonal imbalance.
