Usage
Succimer capsules contain dimercaptosuccinic acid (DMSA), a chelating agent used for the treatment of heavy metal poisoning, particularly lead poisoning. Succimer binds to certain heavy metals in the body and facilitates their elimination through the urine.
It is commonly used for:
• Treatment of lead poisoning, especially in children with elevated blood lead levels
• Chelation therapy for certain cases of heavy metal exposure under medical supervision
• Situations where oral chelation therapy is preferred over parenteral treatment
Compounded succimer capsules may be prepared when customised doses are required, particularly for paediatric patients or individuals who require specific dosing adjustments.

Dosage
Dosage should be determined by the prescribing practitioner based on the patient’s body weight, severity of poisoning, and blood lead levels.
Typical dosing considerations include:
• Treatment is often administered in multiple daily doses during the initial phase, followed by a reduced dosing frequency during the continuation phase.
• Capsules may be taken with or without food.
• In some cases, the capsule contents may be opened and mixed with a small amount of soft food if swallowing capsules is difficult.
Patients should follow the prescribed dosing schedule carefully and complete the full course of treatment as directed.

Mechanisms of Action
Succimer is a chelating agent that binds to certain heavy metals and promotes their removal from the body.
Its mechanisms include:
Chelation of heavy metals
Succimer contains sulfhydryl groups that bind strongly to heavy metals such as lead, forming stable complexes.
Enhanced urinary excretion
The metal–succimer complexes are water-soluble and are primarily eliminated through the kidneys in the urine.
Reduction of toxic metal burden

Succimer has a relatively selective action for heavy metals and generally has minimal effect on essential minerals compared with some other chelating agents.

Contraindications
Succimer should not be used in patients with:
• Known hypersensitivity to succimer or other related chelating agents

Precautions
Succimer should be used with caution in patients with:
• Liver impairment, as liver enzyme elevations may occur during treatment
• Kidney impairment, since elimination occurs primarily through renal excretion
• Haematological abnormalities, including low white blood cell counts
Other considerations include:
• Blood lead levels and clinical status should be monitored during therapy.
• Liver function tests and blood counts may be monitored during prolonged treatment.
• Adequate hydration is recommended to assist with urinary excretion of metal complexes.
Patients should seek medical advice if they experience rash, fever, sore throat, unusual fatigue, or other unexpected symptoms during treatment.
