Piroxicam Capsules
Usage
Piroxicam capsules are used for the management of pain and inflammation associated with musculoskeletal and inflammatory conditions. Piroxicam is a non-steroidal anti-inflammatory drug (NSAID) that may be prescribed to relieve symptoms such as pain, swelling, stiffness, and reduced joint mobility.
It is commonly used in the treatment of conditions including:
• Osteoarthritis
• Rheumatoid arthritis
• Ankylosing spondylitis
• Acute musculoskeletal pain
• Inflammatory joint disorders
Compounded piroxicam capsules may be prepared when customised strengths or dosage adjustments are required for individual patients.
Dosage
The dosage of piroxicam should be determined by the prescribing practitioner based on the patient’s condition, response to treatment, and overall health status.
Typical dosing considerations include:
• Adults: Often administered once daily due to the long half-life of piroxicam.
• Capsules should be taken with food or after meals to help reduce gastrointestinal irritation.
• The lowest effective dose for the shortest duration should generally be used.
Patients should follow their healthcare provider’s instructions and should not exceed the prescribed dose.
Mechanisms of Action
Piroxicam belongs to the class of medications known as non-steroidal anti-inflammatory drugs (NSAIDs).
Its therapeutic effects occur primarily through:
Inhibition of cyclooxygenase (COX) enzymes
• Piroxicam inhibits both COX-1 and COX-2 enzymes, which are responsible for the synthesis of prostaglandins.
Reduction of prostaglandin production
• Prostaglandins are mediators that contribute to inflammation, pain, and fever.
Anti-inflammatory and analgesic effects
• By reducing prostaglandin levels, piroxicam helps relieve pain, swelling, and inflammation associated with inflammatory conditions.
Contraindications & Precautions
Contraindications
Piroxicam should not be used in patients with:
• Known hypersensitivity to piroxicam or other NSAIDs
• History of NSAID-induced asthma, urticaria, or allergic reactions
• Active gastrointestinal ulceration or bleeding
• Severe heart failure
• Use in the third trimester of pregnancy
Precautions
Piroxicam should be used with caution in patients with:
• History of gastrointestinal disease, including ulcers or bleeding
• Cardiovascular disease or risk factors such as hypertension or ischemic heart disease
• Kidney or liver impairment
• Elderly patients, who may be at greater risk of adverse effects
Other important considerations include:
• Long-term NSAID therapy may increase the risk of gastrointestinal bleeding, cardiovascular events, or renal impairment.
• Patients should avoid using multiple NSAIDs simultaneously unless advised by a healthcare professional.
• Regular monitoring may be required during prolonged therapy.
Patients should seek medical attention if they experience persistent stomach pain, black stools, chest pain, shortness of breath, or signs of allergic reaction.
