Mirtazapine Capsules
Usage
Mirtazapine capsules are an oral medication classified as a noradrenergic and specific serotonergic antidepressant (NaSSA), used to treat depression and certain anxiety-related conditions. It is also known for its sedative and appetite-stimulating effects.
They may be prescribed for:
• Major depressive disorder
• Anxiety disorders (off-label)
• Insomnia associated with depression
• Loss of appetite or weight loss (in selected patients)
• Other practitioner-directed psychiatric conditions
Dosage
Dosage should be determined by the prescribing practitioner based on the patient’s condition and response to treatment.
Typical dosing considerations include:
• Usually administered once daily, typically in the evening due to its sedative effects
• Dose may be gradually increased based on clinical response
• Capsules may be taken with or without food
Patients should take the medication consistently as prescribed and avoid abrupt discontinuation.
Mechanisms of Action
Mirtazapine has a unique antidepressant mechanism involving multiple neurotransmitter systems.
Its mechanisms include:
Alpha-2 adrenergic receptor antagonism
Mirtazapine blocks presynaptic alpha-2 receptors, increasing release of norepinephrine and serotonin.
Serotonin receptor modulation
It selectively blocks certain serotonin receptors (e.g., 5-HT2 and 5-HT3), enhancing beneficial serotonergic activity.
Histamine (H1) receptor antagonism
This contributes to its sedative effects and may improve sleep.
Appetite stimulation
Histamine and serotonin effects may increase appetite and weight gain.
Contraindications
Mirtazapine should not be used in patients with:
• Hypersensitivity to mirtazapine or any component of the formulation
• Concurrent use of monoamine oxidase inhibitors (MAOIs) or within the recommended washout period
Precautions
Mirtazapine should be used with caution in patients with:
• History of suicidal ideation, particularly in younger patients

• Sedation or drowsiness
• Increased appetite and weight gain
• Dry mouth
• May impair ability to drive or operate machinery
• Rare risk of agranulocytosis (low white blood cell count)
• Monitor for signs of serotonin syndrome when used with other serotonergic agents
• Patients should seek medical advice if they experience worsening mood, infection symptoms, or unusual behavioural changes
