Usage
Hydroquinone, tretinoin, and triamcinolone acetonide cream is a compounded topical formulation commonly prescribed for the treatment of melasma, hyperpigmentation, and other pigmentary disorders of the skin.
This combination therapy helps reduce excess pigmentation while improving skin turnover and minimizing inflammation. Hydroquinone works to decrease melanin production, tretinoin enhances skin cell renewal and improves penetration of other active ingredients, and triamcinolone acetonide helps reduce irritation and inflammation that may occur during treatment.
This formulation is often recommended for patients with moderate to severe melasma or resistant hyperpigmentation who require a multi-mechanism therapeutic approach under medical supervision.

Dosage
This compounded medication is typically prepared as a topical cream for external use.
Common compounded strengths include:
• Hydroquinone: 2–6%
• Tretinoin: 0.025–0.05%
• Triamcinolone Acetonide: 0.01–0.1%
Typical application:
• Apply a thin layer once daily in the evening to affected areas of the skin, or as directed by the prescriber.
• The skin should be clean and dry before application.
• Avoid contact with the eyes, lips, and mucous membranes.
• Daily use of a broad-spectrum sunscreen (SPF 30 or higher) is recommended during treatment.
Improvement in pigmentation is typically observed after 4–8 weeks, although longer treatment may be required for optimal results.

Contraindications & Precautions
Contraindications
• Hypersensitivity to hydroquinone, tretinoin, corticosteroids, or any component of the formulation
• Pregnancy or women planning pregnancy (due to tretinoin)
• Use on broken, infected, or severely irritated skin
Precautions
• May cause skin irritation, redness, dryness, peeling, or burning, particularly during the early stages of treatment.
• Prolonged use of topical corticosteroids may cause skin thinning, telangiectasia, or steroid-related skin changes.
• Long-term or excessive use of hydroquinone may rarely lead to exogenous ochronosis (blue-black skin discoloration).
• Avoid excessive sun exposure; pigmentation may worsen without adequate sun protection.
• Treatment should generally be limited to the affected areas and used for the duration recommended by the prescriber.
Patients should discontinue use and consult their healthcare provider if severe irritation or allergic reactions occur.
