Gabapentin Capsules
Usage
Gabapentin capsules are an oral medication used primarily for the management of neuropathic pain and certain seizure disorders. It is also commonly used in a range of off-label pain and neurological conditions.
They may be prescribed for:
• Neuropathic pain (e.g., diabetic neuropathy, postherpetic neuralgia)
• Partial seizures (as adjunctive therapy)
• Chronic pain syndromes
• Restless legs syndrome (RLS) (off-label)
• Other practitioner-directed neurological or pain-related conditions

Dosage
Dosage should be determined by the prescribing practitioner based on the condition, patient response, and tolerability.
Typical dosing considerations include:
• Usually administered two to three times daily
• Dose is often started low and gradually increased (titrated) to minimise side effects
• Capsules may be taken with or without food
• Dose adjustment is required in patients with renal impairment
Patients should take gabapentin consistently as prescribed and should not stop abruptly without medical advice.

Mechanisms of Action
Gabapentin is an anticonvulsant and neuropathic pain modulator.
Its mechanisms include:
Binding to voltage-gated calcium channels
Gabapentin binds to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system.
Reduction of excitatory neurotransmitter release
This decreases the release of neurotransmitters such as glutamate, reducing neuronal excitability.
Modulation of pain signalling
By dampening abnormal nerve activity, gabapentin helps reduce neuropathic pain and seizure activity.

Contraindications
Gabapentin should not be used in patients with:
• Hypersensitivity to gabapentin or any component of the formulation

Precautions
Gabapentin should be used with caution in patients with:
• Renal impairment, requiring dose adjustment
• History of depression or suicidal ideation
• Elderly patients, who may be more sensitive to side effects
• Concurrent use of CNS depressants (e.g., opioids, benzodiazepines)

• Risk of respiratory depression, particularly when combined with other sedating medications
