Famciclovir Capsules
Usage
Famciclovir capsules are used for the treatment of viral infections caused by herpes viruses. Famciclovir is an antiviral medication that helps reduce viral replication and may shorten the duration and severity of symptoms.
It is commonly prescribed for:
• Herpes zoster (shingles)
• Genital herpes (herpes simplex virus type 2)
• Herpes simplex infections, including recurrent outbreaks
• Cold sores (herpes labialis)
• Suppressive therapy to reduce the frequency of recurrent herpes infections in certain patients
Compounded famciclovir capsules may be prepared when customised strengths are required for individual dosing needs.

Dosage
Dosage should be determined by the prescribing practitioner based on the type of infection, severity, and patient factors such as kidney function.
Typical dosing considerations include:
• Administered one to several times daily depending on the clinical indication.
• Capsules may be taken with or without food.
• Treatment for acute infections is usually initiated as early as possible after symptom onset to improve effectiveness.
Dose adjustments may be necessary in patients with renal impairment.
Patients should follow the prescribed dosing schedule and complete the full course of therapy as directed.

Mechanisms of Action
Famciclovir is a prodrug that is rapidly converted in the body to penciclovir, the active antiviral compound.
Its mechanisms include:
Inhibition of viral DNA replication
Penciclovir inhibits viral DNA polymerase, an enzyme required for viral replication.
Selective activation in infected cells
The drug is phosphorylated by viral thymidine kinase within infected cells, which allows selective activity against virus-infected cells.
Suppression of viral replication
By interfering with viral DNA synthesis, famciclovir reduces the ability of herpes viruses to multiply, helping to limit infection severity and duration.

Contraindications
Famciclovir should not be used in patients with:
• Hypersensitivity to famciclovir or penciclovir
• Known allergy to any component of the formulation

Precautions
Famciclovir should be used with caution in patients with:
• Renal impairment, as dose adjustment may be required
• Immunocompromised patients, where infections may be more severe
• Elderly patients, who may have reduced kidney function
Other considerations include:
• Adequate hydration may help reduce the risk of renal side effects.
• Early initiation of treatment after symptom onset improves therapeutic effectiveness.
• Famciclovir does not cure herpes infections, but it helps manage symptoms and reduce viral activity.
Patients should seek medical advice if they experience severe rash, allergic reactions, confusion, or unusual fatigue during treatment.

Storage
• Store below 25°C unless otherwise directed.
• Keep capsules in a tightly closed container.
• Protect from excess heat, moisture, and direct light.
• Keep out of reach of children and pets.
Patients should follow the storage instructions provided on the dispensing label supplied by the pharmacy.
