Usage
Clarithromycin suspension is an oral liquid formulation of a macrolide antibiotic used to treat a range of bacterial infections caused by susceptible organisms. It is particularly useful for paediatric patients or individuals who have difficulty swallowing tablets.
It may be prescribed for:
• Respiratory tract infections (e.g., bronchitis, pneumonia, sinusitis)
• Ear infections (otitis media)
• Skin and soft tissue infections
• Throat infections (e.g., pharyngitis, tonsillitis)
• Helicobacter pylori eradication regimens (in combination with other medicines)
Dosage
Dosage should be determined by the prescribing practitioner based on the patient’s age, weight, infection type, and severity.
Typical dosing considerations include:
• Usually administered twice daily, depending on the indication
• In paediatric patients, dosing is typically calculated based on body weight (mg/kg/day)
• The suspension should be shaken well before use

• History of clarithromycin-associated cholestatic jaundice or hepatic dysfunction
Precautions
Clarithromycin should be used with caution in patients with:
• Hepatic impairment
• Renal impairment, where dose adjustment may be required
• Cardiac conditions, particularly QT prolongation or arrhythmias
• Electrolyte disturbances (e.g., low potassium or magnesium)
Additional considerations include:
• Clarithromycin may interact with a wide range of medications via CYP3A4 inhibition
• Monitor for signs of liver dysfunction, such as jaundice or abnormal liver tests
• Use of antibiotics should be appropriate to reduce the risk of antimicrobial resistance
• Patients should seek medical advice if they experience severe diarrhoea, palpitations, or allergic reactions
Storage
• Store below 25°C unless otherwise directed
• Keep the suspension in a tightly closed container
• Shake well before use
• Protect from heat, moisture, and direct light
• Reconstituted suspensions may have a limited shelf life (e.g., 7–14 days)
