Bethanechol Capsules
Usage
Bethanechol capsules are used to stimulate smooth muscle activity of the bladder and gastrointestinal tract. Bethanechol is a cholinergic agonist that increases muscle tone and motility by mimicking the effects of acetylcholine.
It is commonly prescribed for conditions such as:
• Non-obstructive urinary retention, particularly following surgery or childbirth
• Neurogenic bladder dysfunction
• Certain cases of postoperative or postpartum urinary retention
• Occasionally for reduced gastrointestinal motility when clinically appropriate
Compounded bethanechol capsules may be prepared when customised strengths are required or when commercially available dosage forms are not suitable.

Dosage
The dosage of bethanechol should be determined by the prescribing practitioner based on the patient’s condition, response to treatment, and tolerability.
Typical dosing considerations include:
• Usually administered multiple times daily depending on the clinical indication
• Capsules are often taken on an empty stomach (e.g., 1 hour before or 2 hours after meals) to reduce gastrointestinal side effects
• Dosage may be adjusted gradually based on therapeutic response

Mechanisms of Action
Bethanechol is a direct-acting cholinergic (parasympathomimetic) agent that stimulates muscarinic receptors.

Contraindications
Bethanechol should not be used in patients with:
• Hypersensitivity to bethanechol or related compounds
• Mechanical obstruction of the gastrointestinal or urinary tract
• Recent bladder or gastrointestinal surgery where increased motility could be harmful
• Asthma or bronchospastic disease
• Peptic ulcer disease
• Hyperthyroidism
• Bradycardia or significant cardiovascular disease

Precautions
Bethanechol should be used with caution in patients with:
• Coronary artery disease or cardiovascular disorders
• Hypotension
• Epilepsy or seizure disorders
• Parkinson’s disease
• Gastrointestinal inflammatory conditions
Other considerations include:
• Excess cholinergic stimulation may lead to symptoms such as abdominal cramps, sweating, salivation, nausea, or diarrhoea.
• Patients should seek medical attention if they experience severe dizziness, breathing difficulty, or excessive sweating.
Careful monitoring may be required when initiating therapy or adjusting the dose.
