Ranitidine Suspension
Usage
Ranitidine suspension is an oral liquid formulation of an H₂-receptor antagonist used to reduce gastric acid secretion. In compounding practice, it may still be used in selected patients or veterinary settings where appropriate.
It may be prescribed for:
• Gastroesophageal reflux disease (GERD)
• Peptic ulcer disease (gastric and duodenal ulcers)
• Gastritis and acid-related dyspepsia
• Zollinger–Ellison syndrome
• Veterinary use for gastric ulceration or reflux
Note: In many regions, ranitidine availability is restricted due to regulatory concerns. Use should follow current local guidance (e.g., Medsafe NZ).
Dosage
Dosage should be determined by the prescribing practitioner based on patient factors and indication.
Typical dosing considerations include:
• Administered once or twice daily, depending on condition
• Often taken with or without food
• The suspension should be shaken well before use
• Paediatric or veterinary dosing is typically weight-based
Patients should follow the prescribed dosing schedule.
Mechanisms of Action
Ranitidine is an H₂-receptor antagonist.
Its mechanisms include:
Blockade of histamine H₂ receptors
Ranitidine inhibits histamine stimulation of gastric parietal cells.
Reduction of gastric acid secretion
Decreases both basal and stimulated acid production.
Promotion of mucosal healing
Reduced acid levels allow healing of ulcers and inflamed gastric tissue.
Contraindications
Ranitidine suspension should not be used in patients with:
• Hypersensitivity to ranitidine or other H₂-receptor antagonists
• Known allergy to any component of the formulation
Precautions
Ranitidine should be used with caution in patients with:
• Renal impairment, requiring dose adjustment
• Hepatic impairment
• Elderly patients, who may be more sensitive to CNS effects
Additional considerations include:
• May cause:
• Headache
• Dizziness
• Gastrointestinal disturbances
• Rare CNS effects (e.g., confusion) may occur, particularly in elderly patients
• Long-term use may mask symptoms of serious gastrointestinal conditions
• Use should align with current regulatory recommendations
Patients should seek medical advice if they experience persistent symptoms, abdominal pain, or unusual side effects.
Storage
• Store as directed by the pharmacy, often refrigerated (2–8°C) for compounded suspensions
• Keep the container tightly closed when not in use
• Shake well before use
• Protect from light
• Do not freeze
• Compounded suspensions may have a limited shelf life
• Keep out of reach of children and pets
Patients should follow the specific storage and expiry instructions provided on the dispensing label supplied by the pharmacy.
