Pimobendan Suspension
Usage
Pimobendan suspension is an oral liquid formulation used primarily in veterinary medicine for the management of heart disease in animals, particularly dogs. It is classified as an inodilator (inotropic + vasodilator) and is commonly used in congestive heart failure (CHF).
It may be prescribed for:
• Congestive heart failure in dogs, especially due to:
• Dilated cardiomyopathy (DCM)
• Myxomatous mitral valve disease (MMVD)
• Preclinical heart disease to delay progression (in selected cases)
• Situations where a liquid formulation is preferred for accurate dosing

Dosage
Dosage should be determined by the prescribing veterinarian based on the animal’s weight, condition, and clinical response.
Typical dosing considerations include:
• Administered twice daily (approximately 12 hours apart), or as directed
• Usually given on an empty stomach, about 1 hour before feeding
• The suspension should be shaken well before use
• Dosing is typically weight-based
Regular veterinary monitoring is required to assess treatment response and cardiac function.

Mechanisms of Action
Pimobendan has dual mechanisms that improve cardiac performance:
Positive inotropic effect
• Increases the strength of heart muscle contraction by sensitising cardiac myofilaments to calcium
Phosphodiesterase III (PDE3) inhibition
• Leads to increased intracellular cyclic AMP (cAMP)
• Results in vasodilation of blood vessels
Overall effect
• Improves cardiac output
• Reduces preload and afterload
• Enhances circulatory efficiency

Contraindications
Pimobendan should not be used in:
• Animals with hypersensitivity to pimobendan or formulation components
• Conditions where increased cardiac output is inappropriate, such as:
• Hypertrophic cardiomyopathy
• Certain forms of aortic stenosis

Precautions
Pimobendan should be used with caution in:
• Animals with cardiac arrhythmias
• Hepatic impairment
• Severe systemic illness
Additional considerations include:
• Regular monitoring of cardiac function and clinical signs is recommended
• Possible side effects may include:
• Gastrointestinal upset (vomiting, diarrhoea)
• Lethargy
• Changes in heart rhythm
• Dose adjustments may be required based on response
Owners should seek veterinary advice if they notice worsening breathing, lethargy, or changes in appetite.

Storage
• Store below 25°C unless otherwise directed
• Keep the suspension in a tightly closed container
• Shake well before use
• Protect from heat, moisture, and direct light
• Some formulations may have a limited shelf life after preparation
• Keep out of reach of children and animals
Follow the specific storage and expiry instructions provided on the dispensing label.
