Usage
Metronidazole suspension is an oral liquid formulation used for the treatment of anaerobic bacterial and protozoal infections. It is commonly prescribed when patients are unable to swallow tablets or when a customised dose is required, such as in paediatric or veterinary patients.
Metronidazole is effective against a range of anaerobic bacteria and protozoa and may be used to treat conditions such as:
• Gastrointestinal infections caused by anaerobic bacteria
• Protozoal infections, including Giardia lamblia and Entamoeba histolytica
• Bacterial vaginosis
• Dental and oral infections involving anaerobic organisms
• Intra-abdominal infections
• Certain skin and soft tissue infections
The suspension formulation allows for flexible dosing and improved patient compliance, particularly in children or patients with swallowing difficulties.
Dosage
Dosage should be determined by the prescribing practitioner based on the type and severity of infection, patient age, weight, and clinical response.
Typical dosing considerations include:
• Adults: Usually administered 2–3 times daily depending on the indication
• Paediatric dosing: Typically calculated based on body weight (mg/kg/day) and divided into multiple doses

Mechanisms of Action
Metronidazole is a nitroimidazole antimicrobial agent that exhibits activity against anaerobic bacteria and certain protozoa.
After entering susceptible microorganisms, metronidazole is reduced by microbial enzymes, producing reactive metabolites that disrupt cellular processes.
Its mechanisms include:
• Damage to microbial DNA, leading to inhibition of nucleic acid synthesis
• Disruption of bacterial cell function, resulting in cell death
• Selective activity against anaerobic organisms and protozoa
Because aerobic bacteria lack the necessary metabolic pathways to activate metronidazole, the drug is primarily effective against anaerobic pathogens.
Contraindications & Precautions
Contraindications
Metronidazole should not be used in patients with:
• Known hypersensitivity to metronidazole or other nitroimidazole derivatives
• A history of severe allergic reaction to metronidazole-containing medicines
Precautions
• Use with caution in patients with liver impairment, as metabolism occurs primarily in the liver.
• Prolonged therapy may increase the risk of neurological effects, including peripheral neuropathy.
• Patients should avoid alcohol during treatment and for at least 48 hours after the last dose, as this may cause a disulfiram-like reaction (nausea, vomiting, flushing).
• Use cautiously in patients with central nervous system disorders, including seizure disorders.
• Dosage adjustments may be required in severe hepatic impairment.
Patients should seek medical advice if they experience persistent neurological symptoms, severe gastrointestinal effects, or allergic reactions during treatment.
