Piroxicam Suspension
Usage
Piroxicam suspension is an oral liquid formulation used for the management of pain and inflammation associated with musculoskeletal and inflammatory conditions. It is particularly useful for patients who have difficulty swallowing capsules or require individualised dosing, such as paediatric or elderly patients.
Piroxicam is a non-steroidal anti-inflammatory drug (NSAID) indicated for:
• Osteoarthritis
• Rheumatoid arthritis
• Ankylosing spondylitis
• Acute musculoskeletal pain
• Other inflammatory conditions as determined by the prescribing practitioner

Dosage
Dosage should be determined by the prescribing practitioner based on the patient’s condition, age, weight, and response to treatment.
Typical dosing considerations include:
• Often administered once daily due to the long half-life of piroxicam
• The suspension should be shaken well before use to ensure uniform dosing
• It is generally recommended to take the medication with food or after meals to reduce gastrointestinal irritation
• The lowest effective dose for the shortest duration should be used
Patients should follow the directions provided by their healthcare provider and should not exceed the prescribed dose.

Mechanisms of Action
Piroxicam is a non-steroidal anti-inflammatory drug (NSAID) that exerts analgesic and anti-inflammatory effects.
Its mechanisms include:
Inhibition of cyclooxygenase (COX) enzymes
Piroxicam inhibits both COX-1 and COX-2 enzymes, which are responsible for the synthesis of prostaglandins.
Reduction of prostaglandin production
By decreasing prostaglandin levels, piroxicam reduces inflammation, pain, and fever.
Anti-inflammatory and analgesic effects
The reduction in inflammatory mediators helps alleviate joint swelling, stiffness, and discomfort.

Contraindications
Piroxicam should not be used in patients with:
• Hypersensitivity to piroxicam or other NSAIDs
• History of NSAID-induced asthma, urticaria, or allergic reactions
• Active gastrointestinal ulceration or bleeding
• Severe heart failure
• Use in the third trimester of pregnancy

Precautions
Piroxicam should be used with caution in patients with:
• History of gastrointestinal disease, including ulcers or bleeding
• Cardiovascular disease or risk factors (e.g., hypertension, ischemic heart disease)
• Renal or hepatic impairment
• Elderly patients, who may be at increased risk of adverse effects
Additional considerations include:
• Long-term use may increase the risk of gastrointestinal bleeding, cardiovascular events, or renal impairment
• Avoid concurrent use with other NSAIDs unless directed by a healthcare professional
• Patients should seek medical attention if they experience persistent abdominal pain, black stools, chest pain, or shortness of breath

Storage
• Store below 25°C unless otherwise directed
• Keep the suspension in a tightly closed container
• Shake well before use
• Protect from excess heat, moisture, and direct light
• Keep out of reach of children and pets
Patients should follow the specific storage instructions provided on the dispensing label supplied by the pharmacy.
