Monobenzone Cream
Usage
Monobenzone cream is a topical depigmenting agent used for the treatment of vitiligo, particularly in patients with extensive or widespread disease where uniform depigmentation is desired.
It may be prescribed for:
• Extensive vitiligo to achieve permanent depigmentation of remaining normal skin
• Situations where cosmetic uniformity of skin tone is the treatment goal
• Other specialist-directed dermatological uses
Monobenzone causes irreversible depigmentation and is typically used under specialist dermatological supervision.

Dosage
Dosage and treatment regimen should be determined by the prescribing practitioner (dermatologist).
Typical dosing considerations include:
• Apply a thin layer to pigmented skin areas once or twice daily, or as directed
• Apply to clean, dry skin
• Treatment may be continued over several months to achieve desired depigmentation
• Wash hands after application unless treating the hands
Depigmentation develops gradually and requires consistent use.

Mechanisms of Action
Monobenzone is a depigmenting agent that permanently reduces melanin production.
Its mechanisms include:
Destruction of melanocytes
Monobenzone is metabolised into reactive compounds that selectively destroy melanocytes (melanin-producing cells).
Inhibition of melanin synthesis
It interferes with tyrosinase activity, reducing melanin production.
Permanent depigmentation
Loss of melanocytes results in irreversible lightening of the skin in treated areas.

Contraindications
Monobenzone cream should not be used in patients with:
• Hypersensitivity to monobenzone or any component of the formulation
• Non-vitiligo skin conditions, where depigmentation is not indicated
• Patients seeking temporary skin lightening

Precautions
Monobenzone should be used with caution in patients with:
• Localized vitiligo, where depigmentation may not be appropriate
• Sensitive skin, due to risk of irritation
• Psychological considerations, as depigmentation is permanent
Additional considerations include:
• Depigmentation may occur beyond treated areas due to systemic or local spread
• Skin becomes more sensitive to sunlight, increasing risk of sunburn
• Possible side effects include:
• Skin irritation
• Redness
• Contact dermatitis
• Strict sun protection (SPF, protective clothing) is essential
• Patients should be counselled about the permanent nature of treatment
Patients should seek medical advice if they experience severe irritation or unexpected skin changes.

Storage
• Store below 25°C unless otherwise directed
• Keep the container tightly closed when not in use
• Protect from heat, moisture, and direct light
• Do not freeze unless specifically instructed
• Keep out of reach of children and pets
Patients should follow the storage instructions provided on the dispensing label supplied by the pharmacy.
