Ketotifen Capsules
Usage
Ketotifen capsules are an oral antihistamine and mast cell stabiliser used for the management of allergic conditions and, in some cases, asthma prophylaxis. In compounding practice, they are often used for specialist or off-label indications.
They may be prescribed for:
• Allergic conditions (e.g., allergic rhinitis, urticaria, chronic itching)
• Asthma prophylaxis (preventive therapy, not for acute attacks)
• Mast cell–related disorders (e.g., mast cell activation syndromes)
• Other practitioner-directed uses involving histamine-mediated conditions
Dosage
Dosage should be determined by the prescribing practitioner based on the patient’s condition and response.
Typical dosing considerations include:
• Usually administered once or twice daily
• Often started at a low dose and gradually increased to minimise sedation
• Capsules may be taken with or without food
Patients should take ketotifen regularly as prescribed; it is not intended for immediate relief of acute symptoms.
Mechanisms of Action
Ketotifen has dual pharmacological actions:
H₁ histamine receptor antagonism
Blocks histamine receptors, reducing symptoms such as itching, swelling, and redness.
Mast cell stabilisation
Prevents release of inflammatory mediators (e.g., histamine, leukotrienes) from mast cells.
Anti-inflammatory effects
Helps reduce allergic inflammation and hypersensitivity responses over time.
Contraindications
Ketotifen should not be used in patients with:
• Hypersensitivity to ketotifen or any component of the formulation
Precautions
Ketotifen should be used with caution in patients with:
• History of seizures
• Diabetes mellitus (in some formulations)
• Hepatic impairment
Additional considerations include:
• Common side effects include:
• Drowsiness or sedation
• Dizziness
• Dry mouth
• May impair ability to drive or operate machinery
• Effects may take several weeks to become fully apparent
• Should not be used for acute asthma attacks
• Use caution with alcohol or other CNS depressants
Patients should seek medical advice if they experience persistent sedation, unusual symptoms, or worsening condition.
Storage
• Store below 25°C unless otherwise directed
• Keep capsules in a tightly closed container
• Protect from heat, moisture, and direct light
• Keep out of reach of children and pets
